LLC Name

For a Limited Liability Company, company names must contain "Limited Liability
Company," "Limited Company" or the abbreviations "LLC" or "LC.". The
name cannot be the same as or deceptively similar to the corporate name
of a domestic corporation or a foreign qualified corporation.
The Kentucky Limited Liability Company Act, which establishes the guidelines
and requirements for the formation of a Limited Liability Company (LLC) in
Kentucky went into effect July 15, 1994.
The Limited Liability Company must file an annual report with the Secretary of State between January 1 and June 30 of the year
following the calendar year in which the company was organized. Subsequent annual reports must be filed with the Secretary of State
between January 1 and June 30 of the following calendar years.

The address of the initial registered office and the name of the initial registered agent.

A statement that the limited liability company has at least two or more members.

The mailing address of the initial principal office.

A statement that the limited liability company is to be managed by managers or
is to be managed by members.

If the limited liability company is to have a specific date of dissolution. The latest
date on which the limited liability company is to dissolve.
