    Foreign Non Profit Corporation Name
     
    • The name of the corporation must be distinguishable upon the records of the Secretary of State from any other domestic/foreign corporation, domestic/foreign limited partnership, limited liability partnership, limited liability limited partnership, name reservation or domestic/foreign limited liability company registered to do business in the state of Missouri.
    Forms must be accompanied by a current "Certificate of Good Standing/Existence" (within 60 days) from the parent state. This must be an original with original seals and signatures. This certificate can normally be obtained from your Secretary of State or authority which issues corporate charters, for a fee. We do not require, and do not accept, copies of Articles of Incorporation.

    The check should be made payable to Director of Revenue.
     
    • Application for Certificate of Authority containing the following information
      • The name of the corporation and it is organized and existing under the laws of_______
      • The name it will use in Missouri
      • The date of its incorporation and the period of its duration
      • The address of its principal place of business
      • The name and address of its registered agent and office in the State of Missouri
      • The names of its officers and directors and their business or home addresses are as follows (attach additional sheets as necessary)
      • The specific purpose(s) of its business in Missouri
      • Does the corporation have members? (yes or no)
      • If incorporated in Missouri would the corporation be a public benefit _______________ or mutual benefit _______________ Corporation?
      • The effective date of this document is the date it is filed by the Secretary of State of Missouri, unless you indicate a future date
