    LLC Name
     
    • The name of a limited liability company shall include the words, "limited liability company," without abbreviation or shall include one of the following abbreviations: "LLC," "L.L.C.," "limited," "ltd.," or "ltd".
    A limited liability company may be formed in Ohio by any person without regard to residence, domicile or state of organization. The entity is formed when one or more persons or their authorized representatives signs and files Articles of Organization with the Secretary of State.

    The articles must include the name of the limited liability company, and may include an address where interested persons may direct requests for copies of any operating agreement and any bylaws of the limited liability company. Additional provisions may be attached as long as they are consistent with applicable Ohio law. Limited liability companies may be formed for any lawful purpose, including professional services. Articles must be accompanied by an appointment of agent for service of process (R.C. 1705.06) and a $125 filing fee.

      • Effective Date (Optional)
      • The address to which interested persons may direct requests for copies of any operating agreement and any bylaws of this limited liability company
      • The undersigned authorized member, manager or representative hereby appoint the following to be statutory agent upon whom any process, notice or demand required or permitted by statute to be served upon the limited liability company may be served.
      • The name and address of the agent
      • Must be authenticated by an authorized representative
    • Filling Fees
      • Filing Fee $125.00
